Insurance-Based Medical Care
When WellifyCare participates with your health plan and the service is covered, eligible medical services are billed to insurance.
- Initial medical evaluation
- Medical follow-up
- Obesity medicine
- Prediabetes and type 2 diabetes care
- Medication management
- Blood pressure management
- Cholesterol and cardiovascular risk management
- Clinically appropriate testing and referrals
- Eligible RPM, when offered and requirements are met
Deductibles, copays, coinsurance, prior authorization requirements, network status, and other health-plan rules may apply. Verification of benefits is not a guarantee of payment.
Accepted insurance plans will be listed after contracting is confirmed.
WellifyCare Guided Care
Guided Care is an optional self-pay program for structured lifestyle coaching, education, planning, and longitudinal support.
$200/month
Initial four-month commitment. $800 total during the Foundation Phase.
$750 pay in full
Save $50 on the initial four-month Foundation Phase.
Guided Care does not replace health insurance. Medical services are handled separately and may be billed to insurance when applicable.

Medications, laboratory tests, imaging, medical devices, specialist services, and other third-party services are billed separately by the applicable pharmacy, laboratory, facility, or provider unless explicitly stated otherwise.
